Business size 3-fold order form is from approximately 1972. It lists ticket prices (box seat $4.00, other seats $3.50, $3.00, $2.75, $2.50) and the back pictures Mr. Met.

Overall condition is very good with a wrinkle at the bottom of the center panel, some light smudges here and there and a couple small areas of scuffing on the adhesive strip.
